I request that Loyola advances $ to me through my Loyola Express Card Flex Account for the purchase of books and/or necessities, and to charge the amount of the advance to my tuition account.

I will be registered for semester hours this semester.

I will live on campus this semester.

I authorize Loyola University to use scholarship and student financial aid funds from this term to pay any of my obligations which are charges to my Loyola tuition account. Such charges can include such items as tuition, fees, residence hall charges, meals, books, parking fees, fines and debts to Loyola from prior terms. I certify that the use of scholarship and financial aid funds to pay these charges will not deprive me of sustenance, and that I have adequate funds for my living expenses.
I understand that the authorization to use federal Title IV financial aid funds in this way may be revoked at any time before the funds are paid to my tuition account and offset against my charges. If I revoke the authorization without paying the charges associated with the advance, Loyola University will cancel my registration.
I agree to pay all collection costs, including attorney’s fees and court costs, in the event that I do not pay this debt in full and it becomes necessary to place the account for collection.
I understand that if it becomes necessary for Loyola University to exercise its option to cancel my registration, I will not receive a refund or reduction in charges other than those which would normally apply to students withdrawing on that date. I also understand that if my registration is cancelled, I will receive no credit for any classes for which I am registered.

IMPORTANT: Vouchers submitted to Financial Aid by 4:00pm will be processed onto your Express Card by 10am the following morning.

Funds placed on the Express Card will remain on your Express Card until they are used or the student separates from the University. Financial Aid cannot process any refunds.
